You need a referral to begin hospice care services. Referrals may come from a physician or be self-initiated when hospice care is requested by the patient or family.
Our hospice care team meets with patient & family at home, in the hospital, or nursing facility. During this time, we discuss comfort needs, preferences, and support goals to ensure care is aligned with the patient’s wishes.
Once eligibility is confirmed and the patient agrees to hospice care, services may begin after the consent form and required documents are completed.
